Application Instructions

  1. Complete and sign the official rebate application form. Make a copy for your records.
  2. Attach a copy of your sales receipt for the qualifying appliance. The receipt must be dated and separately list the appliance.
  3. Applications for refrigerators and freezers must also include:
    1. A receipt from a recycler showing that the existing unit was properly recycled/disposed pursuant to state law; OR
    2. Signature of an appliance retailer or recycler that the existing unit was received by the retailer or recycler and will be recycled/disposed pursuant to state law by the retailer or recycler.
  4. Mail the completed application form, copy of appliance sales receipt and recycler's receipt (if applicable) to:

Allow 6 - 8 weeks to receive your rebate.
